Salary Range: $113,700.00 - $130,300.00

Position Summary

Under the direction of the Transplant Program Medical Director and Senior Nurse Practitioner, the Registered Nurse – Pre-Lung Transplant Coordinator serves as a specialized clinician and patient advocate responsible for managing the full pre-transplant continuum of care for lung transplant candidates. This role ensures regulatory compliance with CMS Conditions of Participation and UNOS bylaws, coordinates multidisciplinary evaluation and serves as the primary clinical point of contact for patients and families navigating the transplant process. The RN Coordinator plays a critical role in reducing adverse survey findings by ensuring timely data submission and adherence to programmatic and accreditation standards.

Job Responsibilities

• Coordinates comprehensive multidisciplinary pre-transplant evaluations, including scheduling, tracking testing, and consultations.
• Collaborates with CUMC and internal teams to align shared-care visits, imaging, and testing with program protocols.
• Monitors patient progress, proactively resolving barriers to ensure timely completion of evaluation and listing readiness.
• Facilitates communication across referring providers, external institutions, and multidisciplinary teams for seamless care coordination.
• Maintains accurate, up-to-date documentation in the EMR and transplant databases; provides weekly panel updates with clinical leadership.
• Ensures compliance with CMS and UNOS requirements, including patient education, documentation, and performance metric tracking.
• Coordinates financial clearance, including insurance authorization, and escalates complex barriers as needed.
• Supports transplant activation logistics, maintaining knowledge of UNOS allocation policies.
• Provides comprehensive patient and caregiver education, tailored to health literacy and readiness, and connects patients to support resources.
• Serves as the primary patient liaison while coordinating multidisciplinary care, quality initiatives, regulatory documentation, and transplant committee processes.

Education

• Bachelor of Science in Nursing (BSN) required.

Experience

• Minimum 3–5 years of clinical nursing experience in pulmonology, critical care, cardiothoracic, or transplant-related settings.
• Prior experience in solid organ transplant coordination, lung transplant programs, or advanced lung disease management is preferred.
• Familiarity with UNOS policies, CMS Conditions of Participation for transplant programs, and transplant regulatory requirements is desired.
• Experience coordinating care across multidisciplinary teams and managing complex, chronically ill patient populations.

Knowledge, Skills and Abilities

• Advanced clinical assessment and critical decision-making skills in the management of patients with advanced lung disease and transplant-related comorbidities.
• In-depth knowledge of UNOS waitlist management requirements, CMS transplant program Conditions of Participation, and quality assurance standards.
• Highly organized with the ability to manage multiple concurrent priorities, deadlines, and complex patient caseloads.
• Exceptional verbal and written communication skills; ability to communicate complex medical information clearly to patients, families, and interdisciplinary team members.
• Proficiency in electronic medical records (Epic)
• Strong interpersonal skills with the ability to cultivate collaborative relationships across disciplines and with external partners.
• Demonstrates integrity, professional demeanor, and a patient-centered approach in all interactions.

Licenses and Certifications

• Current New York State Registered Nurse License and Registration is required.
• Basic Life Support (BLS) certification issued by the American Heart Association is required.
• Infection Control Certificate required or to be obtained within designated onboarding period.
