Care Coordinator II

Contract Type / Duration: 6-Month Contract

Location: Remote - Kentucky Residents Only

Work Hours: 8-hour shifts beginning at 9:00 or 9:30 AM and ending as late as 7:00 PM EST/CST, depending on department needs

Pay Rate: $20 $23/hour, depending on certifications and experience

Job Summary

We are seeking a Care Coordinator II to support member outreach, care coordination, and preventive health initiatives. This member-facing role focuses on engaging members by phone, helping them access appropriate care and resources, scheduling appointments, and supporting preventive care goals.

The ideal candidate has healthcare or community-based experience, strong communication skills, and the ability to build rapport quickly while accurately documenting member interactions across multiple systems.

Key Responsibilities

• Conduct outbound and inbound calls to engage members and support care coordination activities.

• Educate members about recommended care, benefits, available resources, and next steps.

• Support preventive care outreach, including well-child visits, postpartum care, A1C testing, and breast cancer screenings.

• Assist members with scheduling appointments and connecting with healthcare providers and community resources.

• Coordinate follow-up activities based on member needs and care plans.

• Identify member needs and make appropriate referrals to care managers, disease management programs, and community organizations.

• Support members with social determinants of health (SDOH) resource needs.

• Document member interactions, assessments, referrals, and follow-up activities accurately.

• Maintain member records and ensure information is entered correctly across multiple systems.

• Communicate with care managers, providers, practitioners, and other team members to support continuity of care.

• Participate in team meetings and other member engagement activities as needed.

Top Required Skills

1. Outbound member outreach and healthcare communication - Experience conducting outbound calls, engaging members, and supporting provider or appointment scheduling.

2. Healthcare documentation and technology - Ability to accurately document and chart while speaking with members and navigate multiple computer systems.

3. Care coordination and member support - Experience with appointment scheduling, healthcare/community resources, SDOH support, active listening, and strong verbal and written communication.

Preferred Skills / Nice to Have

• Certified Community Health Worker (CHW) certification.

• LPN experience or credentials.

• Healthcare or community-based support experience.

• Medical or clinical knowledge.

• Knowledge of community resources and social determinants of health.

• Previous remote healthcare experience.

• Experience supporting preventive care or health outreach programs.

• Ability to quickly build rapport with members.

• Strong adaptability, reliability, and organizational skills.

Education Requirement

Required: High school diploma or equivalent.

Certifications

Preferred: Certified Community Health Worker (CHW).

LPN: Preferred.

Training

Training is approximately 3 weeks :

• 2 weeks: Classroom training

• 1 week: One-on-one training

• Training hours: 8:00 AM 4:30 PM EST/CST

• Cameras must be on during training.

• Candidates should be prepared to attend the full training schedule without time off during training.

Additional Information

• Remote position for Kentucky residents only .

• Quarterly in-person team meetings may be held at a Louisville-area hub.

• Cameras are expected to be on for most meetings; candidates should maintain a professional work environment and attire.

• Candidates must have a current smartphone capable of meeting required security standards.

• This is a highly member-focused role with direct interaction supporting healthcare access, preventive care, and member engagement.

• Contract may have potential for extension based on business needs.
